The best treatment for brain tumors in India includes advanced surgical techniques, radiation therapy, and targeted treatments, guided by experienced specialists and modern medical technology for improved outcomes.

Brain tumors aren’t one single thing, they range from slow, benign growths that surgery can completely cure to fast, aggressive malignant ones that need a full team approach with surgery, radiation, chemo, and targeted drugs. India has some of the strongest brain tumor programs in the world right now, experienced teams, modern technology, molecular testing that’s on par with anywhere else, and treatment costs that are a fraction of what the same care would run in the US or Europe. The “best” treatment always depends on the tumor type, grade, size, exact location, patient’s age, overall health, and how the tumor is behaving. Here’s how we approach it in 2025.

Diagnosis Has to Be Spot-On First

Nothing moves forward until we know exactly what we’re dealing with. High-resolution MRI with contrast (and often perfusion, diffusion, or spectroscopy sequences) is the starting point, it shows size, location, blood supply, and whether it’s pressing on critical brain areas. PET-CT or MR spectroscopy helps sort tumours from infection, inflammation, or radiation changes. A biopsy either stereotactic (through a tiny hole) or during open surgery, is usually the final word. That tissue tells us the WHO grade and molecular profile:

  • IDH mutation (better prognosis, slower growth)
  • MGMT methylation (predicts response to temozolomide)
  • 1p/19q codeletion (chemosensitive oligodendroglioma)
  • BRAF V600E, NTRK fusions, EGFR amplification, etc.

These markers decide targeted drugs and immunotherapy options far more accurately than scans alone.

Surgery – Usually the First and Most Important Step

For most brain tumors, surgery is the cornerstone. The goal is maximal safe resection, taking out as much tumor as possible while protecting the healthy brain that controls movement, speech, vision, memory, or balance. Modern tools have made this much safer:

  • Neuronavigation and intraoperative MRI for real-time margin checking
  • 5-ALA fluorescence (tumor glows pink under special light)
  • Awake craniotomy for tumors near speech or motor zones
  • Endoscopic or keyhole approaches for pituitary, skull-base, or ventricular tumors

Benign tumors (meningioma, schwannoma, low-grade glioma) are often cured with complete removal. High-grade ones (glioblastoma, anaplastic astrocytoma) benefit from taking out the bulk to reduce mass effect and improve response to follow-up radiation and chemo.

Radiation – Precise and Tailored

When complete removal isn’t possible or the tumor is radiosensitive, radiation steps in. Stereotactic radiosurgery (SRS/Gamma Knife/CyberKnife) gives high-dose radiation in 1–5 sessions to small or leftover tumors with almost no collateral damage. For larger fields, IMRT/VMAT shapes the dose to avoid a healthy brain. Proton therapy (available at Apollo Chennai, Apollo Delhi, and a couple other centers) is great for tumors near the brainstem, optic pathways, or in kids because it stops precisely at the target and spares more normal tissue.

Chemotherapy and Targeted Therapy – Personalized Based on Markers

Temozolomide is standard for high-grade gliomas, usually given with radiation then as maintenance. But molecular testing has changed everything:

  • MGMT methylated tumors respond much better to temozolomide
  • IDH-mutant tumors grow slower and have better survival
  • BRAF V600E opens dabrafenib + trametinib
  • NTRK fusions respond to larotrectinib/entrectinib
  • EGFR amplification or ALK rearrangements allow targeted drugs or immunotherapy trials

This tailored approach has pushed median survival in glioblastoma from 12–15 months to 18–24+ months in many cases, and some patients do even better.

Tumor Board and Team Approach – The Gold Standard

The best results come when a multidisciplinary team reviews every case: neurosurgeon, neuro-radiologist, neuro-oncologist, radiation oncologist, pathologist, sometimes neuropsychologist. They decide the sequence: surgery first, then chemo-radiation; radiation first for inoperable spots; or targeted therapy upfront based on markers. This team's thinking happens in all good centers now.

Follow-Up and Rehab – Keeping Quality of Life High

After treatment, MRI every 3–6 months tracks response or recurrence. Rehabilitation through physical therapy and speech therapy and occupational therapy teaches patients how to restore their lost abilities.
